What Is All-on-4?
All-on-4 (also called All-on-X or full-arch implants) is a fixed implant solution that permanently attaches a complete arch of teeth to just four strategically placed dental implants. Unlike dentures, All-on-4 restorations do notcome out — they're secured to the implants and function like natural teeth.
The “4” refers to the minimum number of implants used, though some patients receive 5, 6, or more depending on their bone density and bite requirements. The back implants are typically placed at an angle to maximize contact with available bone, often eliminating the need for bone grafting.

Stability and Function
- Traditional Dentures: Rest on the gums and can slip, shift, or click during eating and speaking. Lower dentures are particularly prone to movement. Bite force is reduced to approximately 10-20% of natural teeth.
- All-on-4: Fixed permanently to implants — they do not move. Bite force is restored to approximately 70-90% of natural teeth. Patients can eat steak, apples, corn on the cob, and other foods that denture wearers typically avoid.
Bone Preservation
- Traditional Dentures: Do nothing to stop bone loss. The jawbone continues to resorb (shrink) without tooth roots to stimulate it. Over time, this changes facial structure and causes dentures to fit poorly.
- All-on-4: Implants fuse with the jawbone through osseointegration, providing the stimulation needed to preserve bone density. This maintains facial structure and prevents the “sunken” appearance common in long-term denture wearers.
Comfort and Daily Life
- Traditional Dentures: Must be removed nightly for cleaning and to allow gums to rest. Can cause sore spots, require adhesive, and may feel bulky — especially upper dentures that cover the palate.
- All-on-4: Cleaned like natural teeth (brushing and flossing). Never removed by the patient. No adhesive needed. The palate remains uncovered, preserving taste sensation and a natural feel.
Longevity
- Traditional Dentures: Typically last 5-10 years before needing replacement due to wear and changes in jaw shape. Regular relines are needed every 1-2 years to maintain fit.
- All-on-4: The implants themselves are designed to last a lifetime with proper care. The prosthetic teeth (the visible part) typically last 15-20+ years and can be replaced without affecting the implants.
What About Implant-Supported Dentures?
There's a middle-ground option called implant-supported dentures (also known as snap-on dentures or overdentures). These use 2-4 implants to anchor a removable denture, providing better stability than traditional dentures at a lower cost than All-on-4.
Implant dentures still need to be removed for cleaning, but they eliminate the need for adhesive and significantly improve chewing ability. They're an excellent option for patients who want implant benefits but aren't ready for the full All-on-4 investment.
Who Is a Candidate for All-on-4?
All-on-4 is designed for patients who are missing all teeth in one or both arches, or whose remaining teeth are failing and need extraction. Good candidates typically have:
- Sufficient bone density for implant placement (or willingness to undergo bone grafting if needed)
- Good overall health for implant surgery
- Non-smokers or willingness to quit (smoking significantly impacts implant success)
- Commitment to oral hygiene and regular dental visits
- Frustration with denture limitations or desire to avoid them
One of All-on-4's key advantages is that the angled implant placement often allows treatment even for patients who have experienced significant bone loss — patients who might not be candidates for traditional implants without extensive grafting.

Traditional dentures may be the better choice for patients who:
